Abstract
Background: Breast cancer survivors frequently face fear of progression (FoP), a stressful psychosocial issue that impairs quality of life. However, little is known regarding its prevalence in Malaysia. This study was conducted to determine the FoP prevalence and its associated factors in the local context.
Methods: This cross-sectional study investigated patients’ sociodemographic profiles, clinical factors, coping strategies using the Coping Orientation to Problems Experienced Inventory (Brief-COPE) and FoP using the FoP short-form questionnaire (FoP-Q-SF). FoP associated factors were determined using logistic regression.
Results: Out of 242 respondents, most were Malay, aged ≥56 years old, in advanced cancer stage, underwent breast surgery, received treatment and used religion coping strategy. Majority of them (89.3%) had low FoP level, with a smaller proportion (10.7%) having moderate-to-high FoP level. Higher education (OR=11.83, P = 0.035), being employed (OR=14.85, P = 0.020), absence of comorbidity (OR=6.29, P = 0.028), advanced cancer stage (OR=11.95, P = 0.020) and self-distraction coping strategy (OR=3.11, P = 0.010), were associated with higher FoP level. Meanwhile, positive family history (OR=0.09, P = 0.031), receiving hormonal therapy (OR=0.11, P = 0.034) and positive reframing coping strategy (OR=0.33, P = 0.008) were associated with lower FoP level.
Conclusion:Low level of FoP is experienced by most breast cancer patients. By identifying the associated factors, they might benefit from early detection and referral.
